A liability waiver is required to use the Xtreme Trampolines facilities. Print and sign this waiver and bring it with you at your next visit. If the trampoline jumper is under 18, a parent (or legal guardian)
MUST sign the waiver, but does
NOT have to be present. If you are over 18, then an ID is required to verify your age. We recommend only dropping off trampoline jumpers that are over 14 years old.
Please note, that once we have your waiver on file, you do
NOT need to fill out a waiver again. Since our two locations (Carol Stream & Buffalo Grove) are different companies legally, the waiver is
NOT transferable from one facility to the other.
We do
NOT ACCEPT waivers that have been "cut and pasted" to any document reader, like Microsoft Word. Below is a link to the Adobe reader software, so that you can print out our waiver.
